HOW DO WE SET UP APPOINTMENTS FOR MVAs AT ABSOLUTE?
The assessment (1 hr) and subsequent follow up appointments (30 mins) are always administered by the same practitioners, in a one-on-one setting.
The assessment must always be carried out by either a Physiotherapist or Chiropractor. Massage therapy needs to be approved by your insurance company, and is usually involved as part of a Motor Vehicle Accident Claim. In some cases a patient may see a physiotherapist, chiropractor and massage therapist, depending on the severity of their case and what their primary health care provider determines is necessary.
Although exercise is part of your plan, our practitioners are all manual based, hands-on practitioners.
You will get the same quality and duration of treatment as our private paying patients.

Steps to Start an Auto Accident Injury Claim
Step 1 - Make a Personal Injury Claim
Call your auto insurance company. Make a personal injury claim. They may take a few days, but they'll issue you a claim number and assign an insurance adjuster to your case.
Step 2 - Book an Initial Assessment at Absolute Health & Wellness
We recommend booking an assessment as soon as possible. We'll get you in as soon as possible.
Step 3 - Fill Out Your OCF-1 and OCF-5 Forms
Before you start your treatment, you'll need to fill out two important forms.
OCF 1: Is an overview of your personal details and pertinent details around your auto accident.
OCF 5: This is Permission to Disclose - it's permission for our clinic to communicate with your insurance company
Your adjuster will mail a package to you, which will include this OCF 1 and OCF 5. You can also pick these up at our clinic, or download them here. Download the OCF 1 here and OCF 5 here.
Step 4 - Initial Assessment at Absolute Health & Wellness
Come 15 mins early for the first appointment.
We will need: a photocopy of your driver's license, your completed OCF 1 and OCF 5 (we'll send them to your adjuster), your benefits info you have benefits.
We'll then book you for your follow up appointments.
Lastly, we will submit a treatment plan to your auto adjuster.
