Imaging Requisitions

If you're in need of an X-ray, MRI, CT Scan, ultrasound or any other diagnostic imaging, we can expedite the process for getting requests. 

In all cases family doctors can order tests, but in the even a family doctor is unable or you're wanting to expedite the process, we can work directly with our Nurse Practitioner, Crystal, to arrange for appropriate paperwork. As long as imaging is clinically relevent and our physio/chiro and Nurse Crystal feel imaging is necessary. 

Fee: $50

Discuss with your Physio or Chiropractor. Once you pay the $50 fee at the front desk, we'll get the wheels in motion and Crystal Troup, NP will complete your requisition.

The Process:

A practitioner (physio or chiropractor) need to do your assessment.

If it's determined that imaging may be indicated, you can either to go your family doctor and request, or if you need, we can have Crystal Troup prepare the appropriate requisitions, after collaborating with your physio or chiro.

Once Crystal has completed her review and uploaded any appropriate forms to our system, we will send them to you (email or you can pick up in person).

Fee: $75 (to be paid at the front desk)

Notes Requests

If you need a prescription or note for certain insurance requirements, our Nurse Practitioner Crystal can assist. Notes for Orthotics, Braces, Compression Garments, etc. 

Discuss with your Physio or Chiropractor. Once you pay the $50fee at the front desk, we'll get the wheels in motion and Crystal Troup, NP will complete your note.

The Process:

A practitioner (physio or chiropractor) needs to do your assessment.

If it's determined that a brace/compressions etc may be indicated, you can either to go your family doctor and request, or if you need, we can have Crystal Troup prepare the appropriate notes, after collaborating with your physio or chiro.

Once Crystal has completed her review and uploaded any appropriate forms to our system, we will send them to you (email or you can pick up in person).

Fee: $50 (to be paid at the front desk)

Blood Work and Lab Testing

Sometimes blood work can help us get a deeper understanding of your health concerns. In the event your doctor is unable to requisition certain clinically relevant tests, we can work along side our Nurse Practitioner Crystal to acquire blood work requisitions, so long as your Naturopath and Nurse Crystal believe such testing is clinically indicated. 

The Process:

A Naturopath needs to do your assessment.

If it's determined that certain blood work may be indicated, your Naturopath will discuss it with you. There are always 3 options:

  1. You can go to your family doctor to request testing
  2. you can pay for individual tests at the clinic
  3. you can have a virtual consult with Nurse Crystal (for a $50 fee) and if such testing is indicated she will provide you with a requisition (no additional charge for that testing). 

Once Crystal has completed her review and uploaded any appropriate requisitions our system, we will send them to you (email or you can pick up in person).
